Before participating in any West Haven High School Athletics program, all of the below steps must be completed. If not completed, students will not be able to participate, no exceptions.

1. Parents/guardian must complete the Final Forms online. Instructions on how to use Final Forms can be found here.

2. A physical form must be filled out by a physician/health care professional and turned in to Nurse's office. Students can now sign up to complete a physical through the school-based health center located inside West Haven High School. Please visit the health center for more information/to sign up for an appointment. Physical forms can be found here.

Please remember that participating in West Haven High School Athletics is a privilege, not a right. Please act accordingly.
